Patient Portal

To signup for the portal:

1. Click the Patient Portal button at the bottom of this page.
2. Click the Enroll Now button at the bottom of the portal page.
3. Fill out all the boxes with a red asterisk (*) .
4. Enter the Verification Code that is sent to the registered cell phone number.
5. Enter the email address associated with your account.
6. Create New Password.
7. Agree to Practice consent form.

After completing all these steps you should be in the portal. Please make sure your cell phone number is updated or you will not be able to log in. If you have any question please call the office.
