We accept most major insurance companies including Medicare and Medicaid.

Please contact our office for inquiries about self-pay fees and payment plans.

Billing Notice

• All fees, co-pays, and/or deductibles must be paid at the time of service.
• Payment for any outstanding balances will need to be paid, or arrange payment plan with billing department, before further treatments can be schedule.
• Applicable fees will be charged for completion of letters and forms due to the increasing volume of these requests and the time it takes away from scheduled patient care.
• Patients are personally responsible for payment of services not covered by their insurance plans. It is also their responsibility to check what their insurance plans cover.
